1. Payment Policy & Insurance

• GCC Peptide Lab does not accept insurance or third-party payors.

• All services, including peptide therapies, supplements, consultations, and related treatments, are self-pay only.

• The Client is fully responsible for all payments, regardless of any potential reimbursement from insurance providers.

• All listed prices are exclusive of VAT, unless otherwise stated.

2. Scope of Services & Medical Disclaimer

• GCC Peptide Lab provides wellness, peptide-based therapies, and supportive health services only.

• We do not diagnose, treat, cure, or prevent any disease, illness, or medical condition.

• By agreeing to these terms, you confirm that:

  • You are not seeking medical diagnosis or treatment from GCC Peptide Lab.

  • You understand that services are elective wellness interventions.

The Clinic relies on these representations when agreeing to provide services.

3. Use & Disclosure of Health Information (Privacy Consent)

By signing this agreement, you consent to GCC Peptide Lab using and disclosing your Protected Health Information (PHI) for:

• Treatment coordination

• Payment processing

• Healthcare operations

You acknowledge:

• You have the right to review our Privacy Policy prior to signing.

• GCC Peptide Lab may contact you via phone, voicemail, email, or mail regarding:

  • Appointments

  • Treatment updates

  • Lab results

  • Billing matters

You may request restrictions on the use of your PHI; however, the Clinic is not obligated to agree.

You may revoke this consent in writing, except where disclosure has already occurred. If consent is withdrawn, services may be discontinued.

4. Informed Consent & Assumption of Risk

You understand that participation in peptide therapy, injections, supplements, or related services carries inherent risks.

You confirm that:

• You have fully disclosed your medical history, medications, and lifestyle factors.

• You are voluntarily participating and accept all associated risks.

• GCC Peptide Lab is not responsible for complications arising from undisclosed conditions.

5. Risks & Possible Complications

Potential risks may include, but are not limited to:

• Injection-related reactions (pain, bruising, swelling, infection)

• Allergic or adverse reactions

• Inflammation or irritation at the injection site

• Dizziness, lightheadedness, or fainting

• Medication or supplement interactions

• Improper response to peptide therapy

You further confirm that:

• You are not under the influence of illegal substances at the time of treatment.

• You are not knowingly engaging in treatment that may pose increased risk due to undisclosed conditions.

6. Client Representations & Responsibilities

By signing this agreement, you represent and warrant that:

• You have not withheld relevant medical information.

• You understand GCC Peptide Lab does not provide emergency medical care.

• In case of emergency, you will contact emergency services or visit the nearest hospital immediately.

7. No Guarantee of Results

• Results from peptide therapies and wellness services vary between individuals.

• No guarantees or assurances have been made regarding outcomes.

8. Liability Disclaimer

• GCC Peptide Lab shall not be held liable for:

  • Adverse reactions

  • Complications from treatments

  • Outcomes related to client-requested or custom therapies

• Participation is entirely at the Client’s own risk.

9. Acknowledgment & Consent

By signing below, you confirm that:

• You have read and fully understood this agreement.

• You have had the opportunity to ask questions, and all questions have been answered to your satisfaction.

• You voluntarily consent to receive services from GCC Peptide Lab.

• You accept full responsibility for any risks involved.
